Your employees can use this form to ask you to contribute to LGsuper, instead of completing the ATO’s standard Choice of fund form. - Employer information form
Use this form to provide your details to LGsuper so we can accept your contributions on behalf of an employee or a number of employees. We will then provide you with an LGsuper employer number which you will need to be able to contribute using BPAY or cheque. - Contributions remittance form
Use this form to provide details of payments made on behalf of your contributing employee/s. You will need an LGsuper employer number. If you do not have one yet, please complete the Employer information form above. - New employee information form
This form is not available online - please order a supply by using the above link. Use this form to provide new employee details to LGsuper (if you do not use Employer online) and/or as your record of an employee’s election to pay 6% standard member contributions. - Update of member record form
Use this form to update a contributing employee’s name, address, or details of their new employer. - Change of benefit class or employment basis form
Use this form to let us know when a local government employee has chosen to start paying 6% standard member contributions or has had a change in employment that affects their super. - Advice of salary sacrifice agreement form
Use this form to let us know about a new salary sacrifice agreement for standard member contributions. - Termination advice form
Use this form to let us know that a contributing employee has ceased employment. - Contribution suspension form
Use this form to let us know about approved periods of leave without pay of more than 4 weeks duration and approved periods of paid or unpaid parental leave of more than 4 weeks duration. Do not use this form for any other types of leave. - Queensland local government Employer online access form
